Social Insurance Number
(SIN card or other official
document showing SIN such as
income tax return)
Verification of Shelter Costs
(minimum documentation needed
to verify living arrangements and
associated costs)
School Verification
Dependants 16 and over
Status in Canada
Verification of all Individuals
Proof of Date of Birth
and Identity (any one document
sufficient with birth certificate preferred)
Employment Status
(if applicable – one month of pay stubs,
letter of termination or record of employment)
Health Card Number
(Copy of your card)
Verification of Assets and Debts
(One Month of Bank statements)
Verification of all current Income
 
Please note: This information is required for ALL participants applying
including your spouse/partner/dependant adult children and where applicable your children.
 
Only photocopies are required to be provided to your case manager, you must keep your originals.
Photocopies may be done at your Local Office free of charge. Thank you!
